Laminectomy is still the gold standard in decompression of the spinal canal. However extended laminectomy can produce instability. This in turn can lead to fusion, which I always try to avoid. I have adapted this technique to microsurgical procedures and thus by only doing a tailored resection of bone, I am able to preserve stability. However this type of operation is only indicated in special cases when other techniques are not applicable.
